state law, and Big Dan's Insurance Agency's own good business philosophy,
we are providing you with a full disclosure of our Policies and
Practices regarding nonpublic personal information about you that
we will, or have collected and/or disclosed.
Categories of Information We Collect and From Whom
We collect nonpublic personal information about you from the following
sources; information provided by you on applications and other forms,
information from and/or about transactions by insurance companies,
you, and/or us, information from consumer reporting agencies and/or
the Departments of Motor Vehicles, permitting, or licensing departments
from any state, territory or country, information from medical professionals
and/or medical records relating to claims and/or insurability, individuals
other than those proposed for coverage.

We may disclose the following kinds of nonpublic personal information
about you; minimum amount (necessary and legal) of any information
we have obtained to allow any third party to perform a function
for us related to our business and/or you, minimum amount (necessary
and legal) of any information we have obtained to allow any law
enforcement, government office or department, insurance company,
or insurance institution, and/or credit reporting agency to compile
information (statistical or otherwise), investigate, prosecute,
and/or prevent any criminal activity, fraud, and/or misrepresentation
in connection with you and/or Big Dan's Insurance Agency, minimum
amount (necessary and legal) of any information we have obtained
to allow any medical care institution or professional to verify
coverages and/or benefits for you or anyone on your policy(ies)
with us, minimum amount (necessary and legal) of any information
we have obtained to allow audit of our operations and/or services
by any law enforcement, government office or department, insurance
company, insurance institution, and/or credit reporting agency.
Big Dan's Insurance Agency restricts access to non-public
personal information about you to those employees who need to know
said information in order to provide information, products, and/or
services to you, requested by you, and/or as necessary in the operation
of our business.
Your Right to Access and Amend your Personal Information
You have the right to request access to the personal information
that we have obtained, maintain and may have disclosed about you.
You have the right to know the source of the information and the
identity of any individual, law enforcement agency, government office
or department, insurance company or institution, and/or credit reporting
agency we have disclosed information about you to. You have the
right to view said information in person, or request a copy of it
be sent to you (for which we may charge you a reasonable fee to
cover our costs to provide them to you).
To Request Access to and Amend your Information
To obtain access to your information: Submit a request in writing
to Big Dan's Insurance Agency including your name(s), address(s),
social security number(s), drivers license number(s), policy numbers(s),
telephone number(s) (both current and when you were a customer of
Big Dan's Insurance Agency), and the specific information you would
like access to. Indicate whether you wish to view the information
in our office or have the information sent to you. Currently (and
subject to change without notice) our fee for locating, copying,
and sending documents and/or information is $2.00 per copy of each
document required to fulfill your request. To amend your information:
Submit a request in writing to Big Dan's Insurance Agency including
your name(s), address(s), social security number(s), drivers license
number(s), policy numbers(s), telephone number(s) (both current
and when you were a customer of Big Dan's Insurance Agency), along
with the identity of the document(s) and the disputed information(s)
contained therein. Include the changes requested and any supporting
documentation and/or information substantiating the requested change.
Dispute Resolution
It is agreed that any claim or controversy arising out of or relating
to procurement and disbursement of nonpersonal information by Big
Dan's Insurance Agency and about you will be settled by arbitration
under the rules set up by the American Arbitration Association,
without rights to appeal.
